Keeping your personal information private is very important to me. I do have to take some details from you, such as contact details and name and address. This information is used to provide you with a counselling service, and for quality assurance and statistical purposes. These are legitimate interests under the law.
All records are kept for as long as needed to provide service for you, and as required for insurance purposes. They are kept securely and confidentially and are normally only seen by me.
You do have the right to have access to your records and to tell me if they are inaccurate. If you wish to see them please contact me in writing to request access to your information, providing evidence of your identity, and I will respond to your request within one month. My response will normally be in the form of an appointment listing and a description of any personal data I hold. If you don’t feel that I have dealt with your request appropriately, you can appeal to the Information Commissioner’s Office.
I am a ‘data controller’ under the General Data Protection Regulation (also known as the GDPR)

discussing with supervisors or other professionals in a way to enhance our work together.
If you have been referred through a third party such as through work, they may require the information.
